When your study is complete, you should do the following:
- Click on your project title and in the Left Hand Menu select:
- Forms and Templates
- Then select “HSU IRB Final Report Form.”
- Complete the Final Report Form and re-save on your computer in a Word Document with your name and “Final Report.”
- Return to your project in IRBNet and click “Create a New Package” from the Left Hand Menu.
- Attach your Final Report to the newly created package via the Designer page.
- Electronically “Sign” the package.
- Click “Submit this Package” from the Left Hand Menu
- Select your Organization (Henderson State University)
- Click “Continue”
- Select the Submission Type “Closure/Final Report” and add any additional comments before clicking “Submit.”
